NRP Form Requests 2024 - 2025 Season

Please fill out the sections below to request a NRP form for AA Tryouts for U12 thru U16 Spring Tryouts or B tryouts.

For B tryouts the player would have had to be released from their commitment to WMHA. U14 thru U16 B NRP forms will be available following tryouts. Please fill out this form if released from A tryouts and wish to tryout at a B centre.

U12 & U13 A releases that are Welland residents will attend Welland U13 B tryouts.

Forms will be emailed out to those requesting by an automated system once issued.

Only 1 NRP form per player to AA, and 1 NRP form to B if released from A tryouts.

You must list the centre you wish to tryout out for. Once a NRP form is issued it is final and a 2nd form cannot be issued for AA.

U18 will be available in August for Fall tryouts.
